Full drug history taking


  • Wash hands and introduce yourself
  • Check the patient’s name and DOB
  • Ask if it’s okay to talk to the patient about their medications



  • Start broad:
    • ‘Talk me through the medications you’re taking’
  • Ask what medications they are taking
  • Say that you’ll talk through them one by one
  • Ask if they know what they are all for
  • Ask how much they take of each one and how often/when
  • Ask how often they forget to take their drugs
  • Ask if they have any inhalers
  • Ask if they take any over the counter drugs
    • Paracetamol
    • Ibuprofen
    • Anti-histamine
  • Ask if they take any supplements
  • Ask if they take any recreational drugs



  • Ask if they have any allergies to any drugs
  • Ask what happens when they take that drug


Obtaining medications

  • Ask how they get their medication
    • Collect from the pharmacy
    • Someone else collects
    • Delivered
  • How do they take them
    • Individual packets
    • NOMAD/blister pack
  • Ask about paying for medication



  • How do they find taking their tablets?
    • Problems with swallowing
    • Problems with dexterity
    • Problems with remembering
  • How often do they forget to take their medication?



  • Ask the patient if they have any questions
  • Ask if there is anything they think you’ve missed
  • Summarise what they’ve told you
  • Thank the patient for talking to you
