Female catheterisation

  1. Introduce, explain the procedure and gain consent
  2. Put on apron
  3. Clean trolley or surface and attach a clinical waste bag
  4. Position the patient – needs to be sitting on an inco sheet, but leave her covered with bedclothes at the moment
  5. Open sterile pack, touch as little as possible (only touch corners)
  6. Open all packs – remember to keep the packet of the catheter so stickies can go in notes and on the catheter chart. Pour saline into the bowl.
  7. Ask the lady to position herself with knees up and parted
  8. Put on gloves (thumbs on the outside), put packaging straight in bin
  9. Clean the labia using 3 pieces of cotton, wipe from the front to the back, once on the left, once on right and once between the labia. Use a new piece of cotton each time, and put straight in the bin once finished.
  10. Open instillagel, (can squirt out a little first to prevent initial rush of gel), inform patient that it might feel a little cold, position at the meatus, and insert about 5ml slowly in small pulses. Ensure there is a little gel on the sterile field for the catheter tip later.
  11. Take off gloves and put on second pair, put all rubbish straight in bin
  12. Open the blue sterile sheet, tear a slip in the centre, and place over the patient, with blue side towards the patient
  13. Expose JUST the tip of the catheter and dip in instillagel
  14. Move the tray between the patients legs to catch any spontaneous flow
  15. Explain to the patient what you are about to do, and insert the catheter tip into the meatus
  16. Keep the packet on, and insert catheter up to the bifurcation
  17. Inflate the balloon by depressing the plunger fully, twist off the syringe and throw away
  18. Pull the excess catheter tubing out until you feel resistance
  19. Open the catheter bag, ensure tap is closed, and connect to the catheter tubing
  20. Stretch out the bag, and you should see a flow of urine
  21. Put everything in the clinical waste, and cover the patient back up
  22. Thank the patient for their compliance
